Retirement Payout into Tax Sheltered Annuity Plan
If you would like to transfer your sick days upon retirement into existing 403b and 457b Tax Sheltered Annuity Plans, please click here to complete the DocuSign Retirement Payout Form. After it is submitted, the form is automatically emailed to Benefit Services. Once your request is received, Benefit Services will process the retirement payout accordingly.
